What happens when you call?

A doctor’s order is not needed to make a referral for hospice care, so call us. When you call the information and referral department, they will assign a registered nurse (RN) from the admissions team to come out and visit the patient.

During the first visit, the RN will:

  • Explain the full range of programs and services
  • Explore the patient’s situation and needs
  • Determine the services which best fit the patient’s needs
  • Talk with the doctor about your care needs
  • Have the patient sign a consent form
  • Give you his or her contact information and a copy of A Plan for Life: Personal Journal and Resource Guide
  • Assign the patient to a care team

On the second visit, an RN from the patient’s assigned care team will come out to meet the patient and help develop the first care plan.
