Psychosocial Team Leader - Coral Team



The Psychosocial Team Leader is a qualified Social Worker or counselor who is responsible for coordinating and evaluating the psychosocial component of care for the Interdisciplinary Team. The Psychosocial Team Leader is an active formal mentor and preceptor to new staff on the team, providing teaching, support and guidance.  The Psychosocial Team Leader provides support and leadership in coordinating all team functions in support of the PFCC, including Total Quality Management, supervisory visits and other activities as appropriate.

1. Master's Degree in Social Work, Counseling, or related field from an accredited school.
2. Three (3) years Medical Social Work experience or related field.
3. Prefer Two (2) years Hospice experience.
4. License or license eligible in the state of Florida required.
5. Willingness to attend the preceptor training workshop.
6. Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.  Must be able to provide proof of automobile insurance, a copy of which will be placed in their employee file.
7. Must successfully complete critical thinking skills assessment.
8. Must be willing and able to successfully complete pre-employment computer literacy evaluation and keyboarding skills assessment.
